Form I-90 is used to renew or replace your Permanent Resident Card in many different situations.

Lost, Stolen, or Destroyed — Your card is gone and you need a replacement to travel, work, and prove your permanent resident status.

Damaged Card — Your card is torn, worn, or no longer readable, and needs to be replaced with a clean, valid one.

Name or Information Changed — Your legal name or other details have changed since your card was issued, and your card needs to match.
